Policyholder Portal Account Registration Agreement (Registration Agreement)

By submitting this form, I agree that an administrative account will be established with ShelterPoint Life Insurance Company and all of its subsidiaries (collectively referred to as "ShelterPoint") for my use in:

1. viewing policy information, requesting and/or downloading policy-related documents;
2. viewing the status of premium payments;
3. viewing claims status and claim payment status; and
4. viewing and/or downloading claim reports to which the employer is entitled pursuant to applicable laws and regulations.

As the Primary Administrator on this account, I understand and agree that I am responsible for the actions of individuals I authorize as additional users to manage the insurance policies associated with this account and for all acts or omissions of all users. I understand and agree that any additional user I add to this account must be an employee of the policyholder. I represent that I am authorized to add and manage additional users for this account, and have all necessary authority and approval to take all actions performed by me on this account.

I understand that all additional users will have the same account access and permissions as the primary administrator, with the following exceptions:

• Only the Primary Administrator has the ability to add or delete users on the account; and
• Only the Primary Administrator and HR Administrator user roles will be able to access or view certain claims information.

I understand and agree that if any one of the authorized additional users is no longer employed by the policyholder or no longer requires access to the account, I will immediately remove that user’s access to the account and promptly notify ShelterPoint that the user’s access to the account is no longer active. I further understand and agree that ShelterPoint has no responsibility to monitor or maintain this account, to add or delete users from this account, or to ensure the validity of users on this account, and that I will promptly notify ShelterPoint of any known or suspected unauthorized access to or use(s) of this account, or unauthorized disclosure of any login credentials associated with this account.

As an authorized user for this account, I agree to maintain the confidentiality of my login credentials for accessing and utilizing this account. I will not share my login credentials or allow anyone else to use them.

ShelterPoint retains the right in its sole discretion to cancel any account or any authorized user on any account for any reason including misuse of this site. Access to and use of the account is subject to and governed by ShelterPoint’s website Terms of Use.

ShelterPoint has a Privacy Policy in force in accordance with state and federal regulations. By registering for this online account and clicking the box below, I acknowledge that I have reviewed and accept the terms of ShelterPoint’s Terms of Use, Privacy Policy, and the terms of this Registration  Agreement.
